**Evening appointments are scheduled Monday through Friday until 7 pm as needed in the Hamilton Office.
Hamilton Pediatrics takes pride in offering a range of providers and appointment times to accommodate your schedule. Please note that all children ages 2 through 21 years of age should have a complete physical exam each year. Check with your insurance company to make sure this is a covered service.
We will not be able to complete any health forms unless your child has been examined within one year of the date of the request. We will not give a child clearance to participate in a school sport or extra curricular activities without a yearly complete physical exam.
Babies receive immunizations at well check up appointments according to the following ages:

Insurance and Billing
Please call us for information specific to your insurance company and plan.
Please note the primary insurance for a child is chosen according to the "Birthday Rule". Whichever parent has the first birthday of the year, regardless of the year, is appointed by the insurance companies involved as the primary insurance carrier for the child.
We are required by the insurance companies to bill that insurance company first and the other insurance company as secondary. Please check with your insurance company if you have any questions regarding the "Birthday Rule".
This coordination of benefits is critical to the billing process and will keep us from billing you for services that are covered by your insurance carrier.
We accept checks, cash or credit cards. We can also create a manageable payment plan if necessary. Please contact our Financial Coordinator for details.
